Thank you for signing up for our Mass. It is a Washington State Mandate that all register for Masses.

• Please arrive 15 minutes early due to our check in process. If you arrive late for the Mass you signed up for, you may not have a space.
• Please include the names of all attending the Mass on this form where indicated.
• All attendees are required to take their temperature before they come to Mass verifying that it is 98.6 degrees or below
• All attendees please bring a mask and personal hand sanitizer, we have a limited supply available
• Mass will be shortened and attendees, after receiving the Eucharist, will proceed to their cars, no after Mass socializing
• Walk-ins are welcome after the sign up form is no longer available until we reach capacity
